Axial T2/fluid-attenuated inversion recovery magnetic resonance imaging demonstrating subcortical ischemic vascular changes in (a) a patient with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) and (b) a patient with vascular cognitive impairment due to small vessel disease. Anterior temporal lobe involvement distinguishes CADASIL from small vessel disease due to cerebrovascular risk factors.
